Watermelon is a high glycemic index fruit but it has a low glycemic load. Having a high glycemic index means that the carbohydrates within watermelon are broken down very quickly, resulting in spikes in both your blood sugar and energy (both which are not ideal for diabetics). However, watermelon has a low glycemic load which means that you need to eat a lot of watermelon before it has the aforementioned negative effects. Therefore, diabetics can eat small portions of watermelon at a time without risking significant spikes in their blood sugar levels.
